Short-Term Intensive Weight Loss Program Works for Four Years; Valid Option Seen to Bariatric Surgery

Newswise PHILADELPHIA -- June 12, 2012 -- A study by a team of clinicians and researchers at the Joslin Diabetes Center in Boston has shown for the first time that patients with diabetes who enrolled in a short-term intensive weight management program were able to lose weight and keep it off on their own for four years. The findings suggest an alternative course to bariatric surgery in the fight against type 2 diabetes.

The study, presented as a late-breaking abstract Sunday at the American Diabetes Associations 72nd Scientific Sessions, strongly counters the popular view that people generally cannot maintain long-term the weight loss achieved during lifestyle intervention programs.

The notion that most people in the medical field have is that when you apply intensive lifestyle interventions, the majority of patients gain all or most of the weight back in a year, said Osama Hamdy, M.D., Ph.D., Medical Director of the Obesity Clinical Program and Director of Inpatient Diabetes Management at Joslin and lead author of the paper.

People have been pessimistic, he added. They think they may need bariatric surgery. But we are sending an optimistic message. Think again. There is something else that can work effectively in real-world clinical practice and save money too. This is a very important observation.

The study showed that around 50 percent of the 120 patients who enrolled in the Weight Achievement and Intensive Treatment (Why WAIT) program for 12 weeks were able to maintain an average of 9.5 percent weight loss at four years, while the total group maintained 6.3 percent at four years.

Why WAIT is a multidisciplinary diabetes weight management program designed for clinical practice. The program included a change in diabetes medications to enhance weight reduction, structured dietary intervention with lower carbohydrates and higher protein and meal replacement, an exercise program, with emphasis on strength training and weekly educational and support sessions.

Although subjects in this study were followed for four years, they were on their own after the initial 12-week program. The study found that about half of the subjects did not regain the weight -- on average 24 pounds -- after four years. This group also maintained a significant improvement in their metabolic control as measured by hemoglobin A1c and the improvement in other vascular risk factors like blood pressure and lipids.

It also found that those who maintained at least seven percent of their weight loss at one year were most likely to continue the weight reduction over the long term.

This is very important information, Dr. Hamdy said. It tells us not everyone gains the weight back. In reality, these are similar numbers to the weight loss gained by some bariatric surgeries at that time frame.

Those who had lost at least seven percent of their body weight after one year lost an average of 29 pounds -- or 11.9 percent of their body weight -- at 12 weeks and maintained an average loss of 31.5 pounds -- or 12.6 percent -- at one year.

A fitness expert claims to have identified six different types of so-called bingo wings and a way to get rid of them all.

Brace yourself though, because they dont exactly have the most flattering of names.

While some may suffer from Rump Lumps, others are hiding Double Chubble or, the very worst, Jabba Flabba.

According to research, three-quarters of women hate their bingo wings, the flaps of skin which hang under the triceps and they plague some men, too.

Caused by sagging skin and excess weight, they are almost impossible to shift with ordinary exercise routines.

Lettuce Diets and Weight Watchers? These New Moms Need to Stop

Move over Jennifer Hudson! Jessica Simpson is the new celebrity spokeswoman for Weight Watchers.

Why Jessica Simpson? Because God forbid a new mothers first order of business be to bond with her baby. She has weight to lose and fast!

Its terrible the amount of pressure placed on celebrity mothers and, by extension, non-celebrity mothers to lose baby weight as quickly as possible as soon as they leave the hospital.

Worse, are the celebrities that face the pressure themselves and then turn around and cover magazines promoting their weight loss as an even prouder moment than birthing a healthy baby.

Even Queen Bey has completely fed into this nonsense. She made headlines after telling her fans at a concert that she lost 60 pounds in five months by running on a treadmill and eating lettuce. I hope she was only kidding because that is ridiculous, especially since she is (or was?) breastfeeding.

New mom, actress and author Tia Mowry says that she was encouraged to seek surgery to help her lose her baby weight. At a book signing in DC, she told BellyItchBlog.

Money was being thrown at me to do surgeries and I said no, I want to do it the right way because I know there are moms out there who can relate.

I lost 5 lbs a month which is what your OB/GYN suggest you to do, I would work out 3 times out of the week because moms, who are working moms, who want to spend time with their baby, dont have time to be in the gym 2 hours a day.

And why should they? Its one thing if a person had a baby a decade ago and is still calling the extra poundage post-baby weight, but we really need to give these brand new moms a break.

Studies have shown that most mothers return to their pre-pregnancy weight just a year after giving birth.So why glorify someone who has dropped the weight in five weeks as though that is something to be proud of and aspire toward? Its unrealistic at best and dangerous at worst.

How to safely and healthfully gain weight

It may seem odd that Im writing a column on how to gain weight when so many people trying to do the opposite.

But for some individuals, trying to gain extra pounds and keep them on can be a real challenge.

Reasons for wanting to gain weight include building muscle, feeling healthier and looking better. Some people lose their appetite or experience a change in metabolism due to stress, illness or depression.

Body mass index (BMI) is one gauge to know if youre underweight. Your weigh is considered normal, or healthy, if your BMI is between 18.5 and 24.9. Anything under 18.5 is considered underweight.

BMI is calculated by dividing a persons weight (in kilograms) by his height (in metres squared). You can also use an online BMI calculator to determine your number.

Just as there are health problems associated with carrying too much weight, being underweight has its own risks. Those who are underweight may have weaker immune systems and get sicker more often during cold and flu season. Being underweight can also increase the risk of osteoporosis, fertility problems and nutrient deficiencies.

Not everyone who wants to gain weight is underweight. Many males who strength train want a food plan to increase muscle weight. Others want to gain weight to feel more energetic. And some people simply want to overcome looking what they feel is too skinny.

Theoretically, you need to eat an extra 500 calories a day in order to gain one pound a week. Some people, however, have difficulty gaining weight and need to consume more.

To gain weight healthfully, choose foods that are calorie and nutrient-dense rather than opting for high-calorie foods packed with sugar and saturated (animal) fat.

NEW YORK: When fitness instructor Shirley Archer plays Annie Lennoxs song 17 Again for her indoor cycling class, she urges students to connect with the vitality of their inner 17-year-old legs. When she teaches yoga she rolls out the harp solos.

Experts say exercising to the proper music can boost your mood, kick your workout up a notch and channel the energy of a younger you.

Fitness playlists are extremely important, said Archer, who is also a spokeswoman for the American Council on Exercise. A number of studies show that music can impact mood, emotions, energy levels, even breathing and heart rates.

Archer, author of 12 books on fitness and wellness, including Fitness 9 to 5, said the research comes from music therapy, sports, surgical recovery and gait training studies for people with neurodegenerative issues.

Music is powerful because it stimulates different neural pathways in the brain and taps powerfully into our emotions and our memories, said Archer, who lives in Singer Island, Florida, and Zurich, Switzerland.

Not only can music help people power through workouts, she said, it can distract them from boredom, fatigue and discomfort.

Think about how people who are middle-aged or older find their dancing feet when they hear tunes that were popular when they were in their teens or early 20s, she said.

Also a powerful tool for relaxation, music can help to slow and calm heart and breathing rates, reduce stress and evoke feelings of peace and balance.

A good fitness playlist will match the workout objectives, Archer said. An aerobic workout should start out with uplifting music at a tempo of about 130 beats per minute and build to around 150 BPM.

Studies show that this is a moderate aerobic pace for most people, she said. Think of the tunes Stayin Alive, 9 to 5 or Justin Timberlakes Rock Your Body.
