Double Whammy: Men With Erectile Dysfunction Also Have Higher Risk Of Death – Study Finds

LEUVEN, Belgium Talking about problems in the bedroom is never an easy task for a man. Culturally, men are inundated from an early age with the idea that the ability to perform sexually at a moments notice is a trademark quality of a real man. According to a new study, however, erectile dysfunction may represent a much bigger problem than just embarrassment.

Researchers have concluded that men with erectile dysfunction face an overall higher risk of death, regardless of their testosterone levels.

As both vascular disease and low testosterone levels can influence erectile function, sexual symptoms can be an early sign for increased cardiovascular risk and mortality, comments lead researcher Dr. Leen Antonio, of KU Leuven-University Hospitals in Belgium, in a release by the Endocrine Society.

Lower than average testosterone levels have been linked to a higher death risk in the past among middle-aged and older men, but most of the research on that topic has yielded inconclusive results. Some studies have even found an association between broad sexual dysfunction in men and a higher mortality risk.

So, in an effort to clarify these vague prior findings, the research team this time around utilized data collected as part of a large observational European study that focused on age-related hormonal changes in older men and subsequent health developments. In all, information on 1,913 men were included in the study.

For each man, researchers investigated the relationship between their hormone levels and sexual functioning both at the beginning of the observation period and once again just over 12 years later, assuming of course said participant was still alive.

During the entire follow-up period (12.4 years), a total of 483 participants (25%) passed away. Among those fatalities, researchers discovered that men with normal levels of testosterone yet still suffering from erectile dysfunction had a 51% higher risk of dying than men with no reported sexual problems.

Additionally, men with notably low testosterone levels and sexual dysfunction symptoms had an even higher risk of death than participants with normal testosterone and no sexual symptoms.

The results were broken down in even greater detail. For example, men who reported erectile dysfunction, weak morning erections, and low sexual drive had a higher death risk than men with normal sexual tendencies. More specifically, men suffering from all three of these complaints had a 1.8 times higher risk of dying than men with no symptoms. For men with just erectile dysfunction, that risk of dying was 1.4 times higher.

Overall, men who died tended to have lower levels of free testosterone, or the testosterone that is easily used by the body.

This research is set to be presented at ENDO 2020, the Endocrine Societys annual meeting. It will also be publishedin the Journal of the Endocrine Society.

What isn’t seen isn’t heard: trans and non-binary health amid COVID-19 – The CT Mirror

March 31 was International Transgender Day of Visibility. Minus a few posts online by media outlets, little was said about it this year. As a public health student, I have been wondering how marginalized communities will be negatively affected by COVID-19 and how some communities needs will be overshadowed as healthcare systems become overwhelmed and medical resources are focused on treating coronavirus patients.

One of the communities that will undoubtedly face increased pressure is the transgender and non-binary community as a result of restrictive coronavirus lockdowns, disrupted access to social services, barriers to prescribing and managing hormone therapy medications, and hospital decisions to halt gender-affirming surgeries.

And in some ways, stay-at-home orders and enforced social distancing policies, while they effectively reduce opportunities for viral transmission, also contribute to trans erasure and create more opportunities for the existence of members of the trans and non-binary communities to be ignored, denied or minimized. What isnt seen isnt heard, and only through increased visibility comes equal rights.

Recently, Gov. Brad Little of Idaho signed into law House Bill 500, the Fairness in Womens Sports Act, and House Bill 509, the Idaho Vital Statistics Act. H.B. 500 states that athletic teams or sports designated for females, women, or girls shall not be open to students of the male sex and that disputes can be resolved by inspecting the students reproductive anatomy, genetic makeup, or normal endogenously 19 produced testosterone levels, effectively banning female-identifying transgender athletes from performing on teams aligned with their chosen gender identity.

H.B. 509 prohibits transgender individuals from changing their gender on their birth certificates. These bills originated from complaints strikingly similar to those articulated in a recent lawsuit filed against the Connecticut Association of Schools, Connecticut Interscholastic Athletic Conference and the boards of education in Bloomfield, Cromwell, Glastonbury, Canton and Danbury aimed to block transgender athletes from participating in girls sports.

With more than 40 states that have introduced similar legislation this year, statewide stay-at-home orders and social distancing policies limit the ability for transgender activists to protest and overturn discriminatory legislation such as these bills. And the media focus on COVID-19 developments might allow this type of legislation to fall through the cracks.

Additionally, trans and non-binary workers are three times more likely to be unemployed compared to the general population, and a lack of employer-based insurance results in a disproportionate insurance coverage gap. The pandemic has only exacerbated existing disparities. The International Foundation for Employee Benefit Plans reported that transgender inclusive healthcare benefits are offered to less than one-third of U.S. employees. With widespread furloughing and unemployment in response to the pandemic, even those with insurance benefits that cover hormone replacement therapy (HRT), counseling, or gender-affirming surgery are at risk of losing coverage.

While over 3 million Americans filed for unemployment as a direct result of layoffs caused by COVID-19, applying for unemployment might create unique challenges for trans individuals who might be required to use their deadname, a name assigned at birth that has electively been changed to match their gender identity, to prove their identity on government forms this can undoubtedly be traumatizing. Furthermore, trans and non-binary individuals might be wary to seek unemployment help as waiting in long lines outside unemployment offices might put them at an increased risk of discriminatory violence.

And it is no secret that, as a result of social stigma and discrimination, the transgender population has a higher rate of unstable housing and homelessness. According to the Williams Institute, over 30% of individuals served by drop-in centers, street outreach workers, and housing programsidentified as LGBT. Unfortunately, those who are transgender and homeless are at increased risk of exploitation, violence, and abuse, and may be turned away from shelters as a result of their gender identities. As some homeless shelters close or limit occupancy to stop the spread of COVID-19, this further limits available housing options for unhoused transgender individuals.

Past negative experiences of being misgendered or refused service when seeking care or out of fears of contracting COVID-19 in healthcare settings may cause trans people to forego care. And trans individuals may be reluctant to go to hospitals and clinics to request hormone replacement therapy (HRT) treatment during the pandemic already overburdened hospitals might not have the capacity to provide sufficient social services or hormone treatments.

Furthermore, gender-confirmation surgeries, deemed non-essential and elective, have been delayed or cancelled at many U.S. hospitals due to coronavirus. Interruptions in HRT provision or postponing long-awaited gender-affirming surgeries could augment feelings of isolation and gender dysphoria and amplify psychological distress. The Trevor Projects 2019 survey reported that more than half of transgender and non-binary young people have contemplated suicide and one-third have attempted it.

The minority stress model, a social psychology theory widely adopted in public health, describes how extreme minority stress causes adverse health outcomes among stigmatized minority groups. Consistent stress, discrimination and internalized stigma faced by transgender people increases their risk of suicidality. And as health systems react to treat coronavirus patients and critical services previously available to transgender patients are interrupted or deprioritized, feelings of isolation and marginalization might further increase rates of suicide or affect transgender mental and physical health and wellbeing.

School closures might also pose unique challenges to transgender youth. With campus LGBTQ resource centers and peer support unavailable and the suspension of all school-based social activities, transgender students might feel unsupported. Furthermore, self-quarantining with abusive family members who do not affirm their trans identity can be extremely traumatizing to trans youth.

As one trans commenter pointed out on a recent Buzzfeed article, I have to make a decision about whether to stay in the UK where I study or fly back to the States to be with my parents, one of whom doesnt use my name or pronouns and has refused to do so. With campuses and dormitories closed to suppress viral transmission, some transgender students must return home to unsafe emotionally and physically abusive homes because they are left with nowhere else to go. And once there, due to enforced lockdowns, they potentially cannot leave.

Quarantine and lockdowns may also increase rates of domestic and intimate partner violence perpetuated against transgender individuals. According to conservative estimates, the National Coalition Against Sexual Violence reports that one in three women and one in nine men will experience sexual violence during their lifetime, and these estimates do not accurately capture the increased risk of sexual violence within the LGBTQ community. Lockdowns and social distancing policies, budget cuts to IPV-related social services and mental health counseling, and financial instability due to unemployment all create opportunities for abusers to perpetrate sexual violence and make it more difficult for transgender victims to leave abusive relationships or report abuse.

Lastly, conservative graphs from the Imperial College COVID-19 Response Team and others by the Centers for Disease Control and Prevention (CDC) show frightening estimates that up to 2.2 million Americans mainly older adults, those who are immunocompromised, or those who have other pre-existing health conditions such as diabetes or lung disease might die as a result of contracting COVID-19. Elderly trans individuals at higher risk of contracting COVID-19 might refuse help from aging providers such as meal delivery programs, senior centers, or other social programs because they are afraid of harassment or discrimination.

Furthermore, statistics show the LGBTQ community is 50% more likely to smoke than the general population, potentially resulting in increased susceptibility to respiratory failure brought on by COVID-19, a respiratory illness. Moreover, trans individuals are more likely than the general population to be diagnosed with cancer and HIV, and reduced immune function could put them at a higher risk to contract coronavirus and face harsher symptoms.

Overall, the coronavirus has not just affected the trans and non-binary communities by restricting access to gender-affirming surgeries or HRT, but has effectively reduced their visibility. Fears of being misgendered or mistreated when seeking unemployment benefits or care in healthcare settings may cause trans and non-binary people to forego seeking assistance during this crisis.

Disruptions in hormone replacement therapy (HRT), counseling, or gender-affirming surgeries and self-quarantining with abusive family members may increase psychological distress and can augment rates of suicidality. Widespread unemployment might result in a loss of employer-based insurance coverage of hormone therapy and gender-affirming surgeries and cause an increase in homelessness among this population. And transphobic legislation may pass in the absence of assembled protestors who remain sequestered at home due to public health efforts to stop the spread of the coronavirus.

While COVID-19 has had a pronounced effect on everyone, those most vulnerable, such as those in the trans community, are poised to receive the most collateral damage.

ur Wild Calling: How Connecting With Animals Can Transform Our Livesand Save Theirs – National Parks Traveler

They were young bulls, possibly even siblings, and their own full-fledged, testosterone-fueled combative rut was probably a full year away. Yet back and forth they jostled in our yard, testing each other not 20 feet from the backdoor, their antlers clattering against each other, steam flushing in bursts from their nostrils in the cold fall air as their gangly legs sought leverage in the not-yet-frozen ground.

Driven instinctually to demonstrate their male supremacy, the two gave and took as they shouldered their sheer bulk behind their antlers, searching for the tipping point. For more than an hour they kept at it, slowly crossing our yard, then up and over the small rise between houses, and into our neighbors yard.

Though Ive seen similar battles on nature shows, seeing one waged in person had been unimaginable until that fall day. I had grown up in New Jersey, where the visible wildlife was usually restricted to deer and birds.

But as my life took me west, I encountered more and more wild animals. Ive been treated to elk, bison, and even wolves roaming Yellowstone National Parks northern range, humpback whales in Glacier Bay National Park and Preserve, a 6-foot Western rattlesnake warming itself in the morning sun in the backcountry of Zion National Park, orcas at work schooling salmon in the waters around the San Juan Islands of Washington state.

Not everyone is so fortunate.

Roughly 80 percent of Americans live in urban areas, according to the U.S. Census Bureau. Moose dont, nor grizzly bears, or herds of bison or elk. The nations development forced thewild lifeout, into smaller, more remote parts of the country. For most Americans, that urbanization also pushed away our interaction with wildlife, our connection with the wild side of the world. It is a personal connection, one more important than many of us probably realize.

Richard Louv spent four years pondering the connection he specifically, and the rest of us in general, have with wildlife. In his 2005 bestseller,Last Child in the Woods, Louv painted a disturbing picture of how we're not just losing bits and pieces of our natural landscape to that urbanization. He underscored how we're failing our children by not working harder to introduce them to nature.

Louvs message then was that through urban sprawl, through the mesmerizing and addictive magic and wizardry of the Internet, and through fear of many of the neighborhoods we call home, we were spawning generations of kids who don't know what it's like to have warm mud squish up through their toes as they wade in creeks to catch frogs, or to hear the gentlewhooshingof a forest's canopy under the breath of the wind as they play hide-and-seek.

Today, Louv warns us that for a large segment of society, that disconnect with wildlife is multiplying our societal nature deficit. Especially today, as more and more of America is being told to stay home in a hopeful bid to stanch the viral march of coronavirus, the separation is growing. To better understand that growing void, I reached out to Louv, whose latest work,Our Wild Calling:How Connecting with Animals Can Transform Our Livesand Save Theirs, examines the importance of wildlife to humans.

It may be more necessary than I thought it would be because of the sense of isolation now people have to endure and the loneliness epidemic thats now been squared, Louv said of his book when I asked what inspired it. Our connection to other animals, even if its outside our window, is I think more important than ever.

Growing up in New Jersey, what wildlife there was was largely ubiquitous, not thought provoking. But as I left the East Coast behind, and moved closer to where the wild things are, they became more important to me, took on a larger role in my life. Knowing that there are annual migrations of thousands of caribou in Alaska is important. But more important is actually being present as herds of bison move almost as one across the Hayden Valley in Yellowstone, or to float quietly in a kayak on a finger of Glacier Bay as a humpback whale and her calf circle, occasionally surfacing to exhale a frothy breath in trade for some fresh air.

We gain meaning from these encounters with other animals, Louv told me. And with our relations with our (domestic) companion animals. But these things that happen when were with wild animals, are particularly mysterious.

Mysterious, yes, but it also triggers something within us. At least it does for me. Experiencing wild animals in their natural settings reinforces my own tie to nature. We are descended from prehistoric cultures that lived on, and from, the land, depending on it for their food and shelter. That tie resides within all of us; deeper for some than others. We are wild at heart. And Louv wants us to remember that.

Louv is ahead of most of us in understanding how impactful wildlife can be upon us, if we open our consciousness to them. Of his ten books, four have been specifically about our human connection with nature. Hes spent years analyzing our relationships, and his own, with wild animals. And he fears that our direct connections with wildlife are in decline.

The question I asked inLast Child, and in the others, is what happens to us? What happens to us psychologically, what happens to us physically, what happens to our social intelligence when we disconnect from nature? Louv asked. All of those things are involved. And what happens to our cognitive abilities? It turns out that this disconnect from nature has real implications.

You can see that disconnect in Yellowstone, Glacier and Grand Teton national parks every summer as visitors approach wildlife 2,000-pound bison and mountain goats with dangerous horns much too closely to capture a photo or a selfie. They dont see thattruly wildanimal filling their viewfinder; they envision a zoo creature, domesticated livestock. That disconnect stems simply from thinking park wildlife are somewhat tame. But the disconnect with the wild side of nature is deeper these days, and affects our understanding and appreciation of the significance of a bison herd, or a wolf pack, or a pod of orcas. For Aldo Leopold, an ecologist and nature writer in the early part of the 20th century, it took the death of a wolf to understand the connection.

We reached the old wolf in time to watch a fierce green fire dying in her eyes. I realized then, and have known ever since, that there was something new to me in those eyessomething known only to her and to the mountain. I was young then, and full of trigger-itch; I thought that because fewer wolves meant more deer, that no wolves would mean hunters paradise. But after seeing the green fire die, I sensed that neither the wolf nor the mountain agreed with such a view.

Whether we actually realize our disconnect with wildlife is something each of us has to mull. But at the end of the day, Louv believes we long for the connection.

"I think its rooted in a deeper loneliness. Which is species loneliness. The urban parks, with the highest benefit for the human psychological health, turn out to be the parks with the highest biodiversity, he told me. I dont think thats an accident. We are desperate as a species, not just as individuals, but as a species to not be alone in the universe, to not feel alone in the universe. Why else would we look for Big Foot? Why else would we look for life on other planets, intelligent life, when people like Stephen Hawkin have told us that it might not be a good idea to find?

As the human culture, we are wired to search for other life, Louv said, even when the life might be bison, or wolves, or moose.

Were surrounded by a larger family of life. We are surrounded by a great conversation going on all around us, wherever we are. Even in the densest urban neighborhood, he went on. The more we learn about other animals, the more we learn about their sentience, the more we learn about their intelligence, the more we learn about their ability to communicate with each other, and with us, between species, not only within their own species. And we can tap into that, even if were locked inside a house. If we pay attention.

Im fortunate for where I live, that I may come into contact with wildlife in the wild. Those two young bull moose tussling in the backyard reminded me of my fortune, and that wild connection.

If life is a road, then we look for exciting or comforting mile markers every year.

Here, in the Big Apple, we circle baseball's opening day as a portal into April, when spring waves its wand over the Northeast, when we can put the cold, snow, and slush in our rearview mirror. Instead of scowling at trucks plowing a wall of muck against your car, you see trees popping with flora. Instead of grim forecasts of frigid weather, you pour over projected starters, run lines, or fantasy lineups. Instead of heavy coats and wool hats, you're ready for baseball caps and leather gloves.

Today was supposed to be that day, the first day of baseball, when we put down shovels and picked up bats, when the monotone of the weatherman is replaced by the wisdom of Keith Hernandez or Ron Darling or David Cone, breaking down balls and strikes, batters and pitchers, for the next six months.

If we are creatures of routine, then there's nothing routine about the sports landscape in general, or Major League Baseball in particular. Right now, the owners and players are trying to hammer out a truncated 2020 season, with the bell for opening day to ring in June. They are working on the granular details of games played, games paid, and service time.

You don't care how the money you spend is chopped up by the bean counters of MLB. You're not versed in the nuance of labor talks. Why should you be? We want to watch Gerrit Cole throw gas past batters in the Bronx. We want Aaron Judge to get healthy and hearty enough to reclaim his crown as Gotham's best baseball player.

Some of us have taken baseball for granted.I have. I jumped on the NFL's broad back and rode it for the last 20 years, often at the expense of MLB. If the Yankees or Mets aren't in the playoffs, my small brain embraces football. I joined the crowd that crowed baseball has too many games. Get it back to 154, the number played by the Boys of Summer. We should be shaving, not adding, the number of teams that qualify for October.

Right now, I'd take all of those arguments and concede them. There's no such thing as too much baseball. This proposed June start jars our ancient sensibilities. Baseball is our bedrock in this rocky world.

There will be certain signs or stats intended to tell us that we are close to whipping this tornadic virus that's been tearing through us.They talk about masks and gloves and sanitizer and ventilators, about flattening curves. Androgen receptor stops tumor growth in the most common form of breast cancer – Newswise

WASHINGTONResearchers say they have found a viable new therapeutic strategy for estrogen receptor-positive (ER+) breast cancer, even cancers that are resistant to current standard of care treatments. Their new preclinical study was accepted for presentation at ENDO 2020, the Endocrine Societys annual meeting, and publication in a special supplemental section of the Journal of the Endocrine Society.

In various models of ER+ breast cancer, the most common form (about 75% of cases) of breast cancer, the researchers found that activating the androgen receptor decreased tumor growth, said Wayne Tilley, Ph.D., the studys principal investigator and a professor at the University of Adelaide in Australia. The androgen receptor is a protein in cells in men and women to which testosterone and other male reproductive hormones called androgens bind to elicit their effects, he explained. Importantly, androgens are found in women as well as men.

There is a prevailing assumption that the androgen receptor promotes malignancy in breast cancer, as it does in prostate cancer, Tilley said. Our study demonstrates that this is not the case for ER-driven breast cancer. Rather, the androgen receptor acts as a tumor suppressor.

ER+ breast cancer needs new drug treatments because as many as 35% of women with this type eventually become resistant to current hormone therapies, Tilley said. He noted that androgen receptor-stimulating drugs are under investigation for various diseases. The researchers tested one of these drugs, enobosarm, from a drug class called selective androgen receptor modulators, or SARMs, in models of ER+ breast cancer. These studies used ER+ breast cancer cell lines that are commercially available as well as tissues taken from breast cancer patients, including those resistant to current therapies. Some cell lines and human tissues were grown in culture dishes, and other cell lines and patient tumors were transplanted into mice to create patient-derived models.

In all models, the researchers found evidence that stimulating androgen receptor activity with enobosarm stopped tumor growth. From the different breast cancer models, they derived a gene signature of androgen receptor activity, which predicted cancer survival in participants in large studies of ER+ breast cancer. This androgen receptor signature outperformed other breast cancer prognostic signatures, he said.

When the investigators treated ER+ breast cancer tissues with enobosarm combined with current standard of care breast cancer drugs such as tamoxifen that target the estrogen receptor, Tilley said they observed better reduction in tumor growth than with the standard drug alone. Enobosarm is nonsteroidal and, according to Tilley, other researchers have found it to be safe in patients. Importantly, it does not cause women to develop male characteristics.

Treatment of ER+ breast cancer with an androgen receptor activator drug could be immediately tested in women, Tilley said.

This study received funding from the National Health and Medical Research Council and the National Breast Cancer Foundation, both in Australia. Tilley reported receiving a research grant from the maker of enobosarm, GTx Inc., now Oncternal Therapeutics.
