You can manage your weight with the energy balance equation. This equation consists of three conditions: energy balance (the calories you take in match the calories that you expend or burn), positive energy balance (you eat more calories than expended), and negative energy balance (where more energy is expended than you take in).

Theoretically, if you are in energy balance, you will not lose or gain any weight. In a positive energy balance, you will gain weight, while in a negative energy balance, you will lose weight.

Either way, whether positive or negative energy balance, numerous changes occur to your bodycellularly, metabolically, and hormonallyto facilitate weight gain or loss. As an example, lets consider extreme weight loss where morbidly obese people are placed on medically supervised diets as an alternative to gastric bypass surgery. Morbidly obese is defined as a body mass index (weight to height ratio) above 40, or having over 100 excess pounds (lbs) of fat. In this example, a female who is five feet tall might weigh 205 lbs or more.

When put on very low-calorie, medically monitored diets of 800 calories per daywhich believe it or not is the common medical treatment at bariatric centers in the United Statesthe individuals lost a massive amount of weight. On average, about 40 lbs or more is lost in an approximate 12-week treatment period.

Here is the problem, though. That weight loss is not only coming from fat. In this example, the 40-lb weight loss is usually about 75% fat and about 25% muscle.

That 25% loss of muscle mass is not desirable; and for this reason, new research, including my own, seeks ways to modify these medically supervised, very low-calorie diets to get the most weight loss from fat while keeping as much muscle as possible, Professor Ormsbee said.

Keep in mind that an 800-calorie diet is only recommended under medical supervision. For the average person, Professor Ormsbee suggests dietary strategies to improve weight loss without sacrificing muscle.

This includes regular exercise and some specific dietary habits, such as increasing protein intake. Regardless, when it comes to energy balance, we should pay attention to how many calories we eat and drink to begin losing or gaining weight, and the patterns may surprise you.

Lets start with a simple question: How many calories do you need to cut from your diet to lose weight? Taking it one step farther, how many calories will it take to lose just one pound?

For years, textbooks and experts have asserted that one pound of fat is equivalent to 3,500 calories. Thus, if you aim to lose one pound of fat per week by modifying your diet alone and not considering exercise, then you would eat 500 less calories per day for one week. Thats pretty easy to wrap your mind around; just skip a daily dessert or an extra serving at dinner.

You might start thinking that if you eat 500 calories less per day and lose one pound per week, you could up this to a reduction of 1,000 calories per day and lose two to three lbs per week. However, the more is better mantra does not always suit us well with diet and exercise.

Recall the example about weight loss coming from both fat and muscle. It turns out that if your energy deficit is over about 1,000 calories per day, it is generally not well tolerated. This is due to many factors, but primarily when you drop your calories by too much, you will also lose musclewhich you do not want.

Too great a calorie deficit also runs a greater risk of missing out on key nutrients, like your vitamins and minerals, and you would likely see your energy levels drop and your fatigue increase. Not to mention, youll probably feel hungry.

One myth that has arisen from this 3,500-calorie rule is that weight will simply continue to drop at the same rate, otherwise known as the linear model of weight loss. In an article in the International Journal of Obesity, the authors studied the accuracy of the 3,500-calorie rule. As it turns out, the rule actually overestimates weight loss, meaning that 3,500 calories may translate to less than one pound lost.

Thus, an update was made. The new model for weight loss, called the thermodynamic model or simply the dynamic model of weight loss, takes into consideration your baseline body composition, age, height, sex, and degree of calorie restriction.This model results in a curvilinear pattern of weight loss over time rather than the traditional linear model of weight loss, and is much more accurate in actual trends for weight loss.

In anotherstudy,473 overweight subjects were placed in randomized controlled groups and half consumed ginger supplements to see what the effect of ginger would be on weight loss. "Ginger intake reduced body weight, waist-to-hip ratio ... fasting glucose and insulin resistance" and helped to boost HDL "or good"cholesterol, according to the findings.

Another studylooked at the effects of taking dietary supplements for green tea, capsaicin and ginger extracts especially what impact these three supplements would have on weight loss. After 8 weeks on the supplements, 50 overweight women"had beneficial effects on weight, BMI, markers of insulin, metabolism." So while ginger was part of the study, it appears to be beneficial both on its own and taken together with green tea and capsaicin(found in chili peppers).

Ginger supplements have been shown to reduce the level of fasting blood sugars, whiich has implications for weight loss and diabetes or pre-diabetes. Inastudy, 41 subjects with type 2 diabetes were assigned to take ginger or placebo for 12 weeks. Results found that"ginger supplementation significantly reduced the levels of fasting blood sugar and A1c, which is a significant measure of how well insulin is working to helpregular blood sugar and keep your body healthy."It seems that oral administration of ginger powder supplement can improve fasting blood sugar, hemoglobin A1c... in type 2 diabetic patients," the study found.

How much would the government have to pay you to take the Covid-19 vaccine?

For most of us, its a silly question. Recent polls show that around 60% of Americans plan to get vaccinated as soon as possible and need no monetary incentive to do so. The problem is that a 60% vaccination rate may not do the trick. Dr. Anthony Fauci said this week that to keep the virus from finding new hosts, social distancing should continue until 75% to 80% of the population is vaccinated.

With several vaccines on the verge of being approved, talk has understandably turned to distribution. Who should be at the head of the line, health-care workers or elderly residents of long-term-care facilities? Do we give it first to the people most likely to die from Covid or to the people most likely to spread it? At some point, these questions will have to be addressed, but they are all secondary to whether we can get enough people vaccinated.

At this point, we still dont even know how well the vaccines will work. We know how they fared in clinical trials, but previous vaccines have proven to be less effective outside clinical settings. People who enroll in clinical trials tend, among other things, to be healthier than the public at large and easier for doctors to observe. But assuming that the vaccines are effective, how do you convince enough people to get inoculated at a time when public trust in government institutions is at historic lows?

One way to go is using carrots instead of sticks. Telling people they cant fly or send their kids to school may work in some cases, but theres evidence that paying people to get vaccinated might be more effective. In a typical year, less than half of American adults bother to get flu shots. But a 2015 study conducted by economists at Swarthmore College found that offering people a $30 reward was enough to increase vaccination rates by 12 percentage points.

The Pick 'n Mix Diet

Often around this time of year people tend to overindulge a little in food and drink and with COVID-19 lockdowns, some of us have also been getting less exercise, a combination that also leads to weight gain. And that is something that is associated with poor health outcomes if we do get infected with COVID-19.

The usual solution to weight gain is to go on a diet but many people find it difficult to stick to more than a few weeks because most diets restrict food choices and it is easy to get fed up (pardon the pun) with eating the same limited food items. If you go on a keto diet you start to miss your carbs and if you go on a zero fat diet you miss many other items that you normally like to eat. And another downside is that because of their restrictions these sorts of extreme or fad diets are not good for your health because you can develop micronutrient deficiencies.

But there is a solution: dont stick to just one boring diet but change your diet every week.

That is where the The Pick n Mix Diet comes in and now is the time for you to take up this new revolution in dieting: Use multiple diets and add some easy physical activities to burn off that excess fat. By changing the diet weekly you don't get bored with it, you have fewer food cravings and are more likely to stick to your weight loss plan. Also, this approach ensures that your diet is more varied and healthier than any single diet plan.

The great thing about the diet part of my weight loss plan is that it does not require sticking with the same boring diet for week after week. So you can wave goodbye to food cravings.

In my book I describe a novel and effective weight loss plan that anyone who is mobile can do. The diet uses sensible, varied, non-extreme dieting (i.e., no fad diets) combined with enjoyable and exhilarating (but not exhausting) exercise that should see you lose around 10 kilograms (22 pounds) in 10 weeks. The great thing about the diet part of my weight loss plan is that it does not require sticking with the same boring diet for week after week. I utilize a variety of different, but equally effective diets that can be changed on a weekly basis. And none of the exercises I recommend should leave anyone with aching muscles or feeling sick or tired. I have also devoted a chapter to describing meal ideas and recipes so you dont need to learn about how the diets work (although that is all explained in the book); you can just follow the weekly meal plans. You can also pick and mix your diets and activities to personalize your weight loss plan to suit your own preferences.

Numerous scientific studies show that the most effective, safe and healthy way to lose excess weight is to both eat less and exercise more. There is no need for fad diets, fad exercise regimens or fatiguing high intensity or interval exercise. Just light or moderate intensity aerobic exercise which is scientifically proven to maximize fat burning. This means you can combine multi-diet dieting with some easy physical activities to burn more fat without hunger, food cravings or tiredness. You can lose weight and get fitter and healthier at the same time.

The Pitch Perfect actress credited walking as her favorite safe and free exercise.

Wilson also worked with personal trainers, focusing on HIIT and mobility workouts.

Just days after revealing that she has officially reached her goal weight one month early, Rebel Wilson got candid with her fans on Instagram, answering their questions and sharing the tips that helped her lose 60 pounds this year.

In a new video shared to IGTV, the 40-year-old actress opened up about the methods that truly made 2020 her self-proclaimed Year of Health.

Her biggest tip? Start walking!

I know Im in a lucky position, I do have access to really amazing personal trainers. But I want you guys to know that the majority of exercise Ive done this year has just been me going out for a walk, she said. No matter where she was in the world, Wilson said she made sure to get her walks in. That is free, you can do it, its safe.

Walking is the best way for mefor my body typeto metabolize fat, she added. On the physical side, my biggest tip, guys, is get out there and walk. If you can do an hour, thats what I like to do, when I go walking, and it is the best. Just get your body moving.

To keep moving, Wilson loves to listen to a motivational podcast while walking or hiking. I never thought I would like hikingwalking uphill. Who would have thought that would be a fun activity? But freak, its good! she said. Get out in nature. Get air into your lungs. I really, really love it, so I do it now all the time.

Throughout the year, Wilson has been keeping fans updated about her progress on Instagram. In addition to hiking, the actress would wake up early for morning sprint workouts, and she also enlisted the help of personal trainers Gunnar Peterson and Jono Castano Acero.

Acero opened up to Today about Wilsons fitness routine, which included high intensity interval training (HIIT), mobility moves, resistance training, and recovery days. He explained that her typical week would look something like this:

In her Instagram video, Wilson opened up about what motivated her to kick off her weight loss journey in the first place. The actress admitted she suffers from emotional eating, which she still struggles with from time to time.

The old patterns still come up, she said. Maybe not every day. But if Im worried or anxious or whatever, it still comes up for me. Its not something thats just going to disappear, its just something I have to manage now moving forward. (She reportedly started following the Mayr Method to boost her nutrition goals.)

Its just about how you manage it and what you can do to try to replace your unhealthy habits with healthy ones, she said.

Its clear shes been committed to her health this past year. She's been working hard, and it shows. Go, Rebel!

Saxenda (liraglutide) injection 3 mg becomes the first FDA-approved therapy to treat obesity in adolescents in more than a decade

PLAINSBORO, NJ, USA I December 4, 2020 I Novo Nordisk today announced that the U.S. Food and Drug Administration (FDA) approved an updated label for Saxenda(liraglutide) injection 3 mg for use in the treatment of obesity in adolescents (1217 years) with a body weight above 60 kg and an initial body mass index (BMI) corresponding to 30 kg/m2 or greater for adults, as an adjunct to reduced-calorie meals and increased physical activity. Saxenda was approved in 2014 for chronic weight management in adults with a BMI 30 kg/m2, or 27 kg/m2 with at least one weight-related comorbidity, as an adjunct to a reduced calorie meal plan and increased physical activity.1

Over the last 20 years, the global prevalence of children and adolescents with excess weight has doubled from 1 in 10 to 1 in 5.2Research also shows that when both parents have excess weight, 80% of their children will have obesity.3However, current treatment options for this population are limited, highlighting a considerable and growing need for additional treatment strategies.4

"New options to treat adolescents who live with obesity can bring much-needed hope to families and help address this growing epidemic," said Dr. Aaron Kelly, Professor of Pediatrics and co-director of the Center for Pediatric Obesity Medicine at the University of Minnesota. "With up to 90 percent of adolescents with obesity likely to have it as adults and thus at increased risk for developing weight-related complications, it's important to address weight care and offer support early on.3,5I'm encouraged that healthcare providers now have another tool in developing a personalized, complete care plan to help adolescents lose weight and keep it off."

The safety and efficacy of Saxenda as a treatment for adolescents with obesity is supported by data from a phase 3a trial published earlier this year in the New England Journal of Medicine. The 56-week clinical trial investigated the effects of Saxenda compared to placebo for weight management in 251 patients aged 12-17 living with obesity as an adjunct to lifestyle therapy, defined as counselling in healthy nutrition and physical activity for weight loss. In the trial, the primary endpoint was change from baseline in Body Mass Index (BMI) Standard Deviation Score (SDS) at week 56.6

The data demonstrated a significant reduction in BMI-SDS, as well as reductions in BMI, mean body weight, and other weight-related endpoints vs. placebo in adolescents with obesity when using Saxenda as an adjunct to lifestyle therapy. Adverse events seen in an adolescent population were similar to those observed in adults. The most common adverse reactions were gastrointestinal events, including nausea, vomiting and diarrhea.6

"The rise in adolescent obesity is contributing to a public health crisis, and it poses a real challenge for healthcare professionals due to the limited treatment options available," said Mads Krogsgaard Thomsen, executive vice president and chief scientific officer of Novo Nordisk. "We are proud to be able to offer a new treatment option for adolescents with obesity and their families in the US, as the FDA approval marks another significant milestone for Saxenda."

What is Saxenda?

Saxenda (liraglutide) injection 3 mg is an injectable prescription medicine used for adults with excess weight (BMI 27) who also have weight-related medical problems or obesity (BMI 30), and children aged 12-17 years with a body weight above 132 pounds (60 kg) and obesity to help them lose weight and keep the weight off. Saxenda should be used with a reduced calorie diet and increased physical activity.

Important Safety Information

Do not share your Saxenda pen with others even if the needle has been changed. You may give other people a serious infection or get a serious infection from them.

What is the most important information I should know about Saxenda?Serious side effects may happen in people who take Saxenda, including:

Possible thyroid tumors, including cancer. Tell your health care professional if you get a lump or swelling in your neck, hoarseness, trouble swallowing, or shortness of breath. These may be symptoms of thyroid cancer. In studies with rats and mice, Saxenda and medicines that work like Saxenda caused thyroid tumors, including thyroid cancer. It is not known if Saxenda will cause thyroid tumors or a type of thyroid cancer called medullary thyroid carcinoma (MTC) in people.

Do not use Saxenda if you or any of your family have ever had MTC, or if you have an endocrine system condition called Multiple Endocrine Neoplasia syndrome type 2 (MEN2).

Tell your health care provider about all the medicines you take, including prescription, over-the-counter medicines, vitamins, and herbal supplements. Saxenda slows stomach emptying and can affect medicines that need to pass through the stomach quickly. Saxenda may affect the way some medicines work and some other medicines may affect the way Saxenda works. Tell your health care provider if you take diabetes medicines, especially insulin and sulfonylurea medicines.

How should I use Saxenda?

What are the possible side effects of Saxenda?Saxenda may cause serious side effects, including:

The most common side effects of Saxenda in adults include nausea, diarrhea, constipation, vomiting, injection site reaction, low blood sugar (hypoglycemia), headache, tiredness (fatigue), dizziness, stomach pain, and change in enzyme (lipase) levels in your blood. Additional common side effects in children are fever and gastroenteritis.

About the phase 3 trial (NCT02918279)

The trial investigated the effect of Saxenda (liraglutide 3.0 mg or maximum tolerated dose) compared to placebo for weight management in 251 adolescents (aged 12 to <18 years) living with obesity as an adjunct to lifestyle therapy.6 The trial included a 12-week run-in period of lifestyle therapy, a 56-week treatment period (including dose escalation over 4 to 8 weeks) on Saxenda or placebo and a 26-week follow-up period without Saxenda or placebo.6 All participants received lifestyle therapy beginning with the run-in period and during the 56-week treatment period and 26-week follow-up period.6 The phase 3a trial was a post-marketing requirement of the FDA under the Pediatric Research Equity Act (PREA), which aims to ensure treatments are safe and effective for children and adolescents.7,8

About SaxendaSaxenda (liraglutide) injection 3.0 mg is a once-daily glucagon-like peptide-1 (GLP-1) receptor agonist with 97% similarity to naturally occurring human GLP-1, a hormone that is involved in appetite regulation and food intake.1 Like human GLP-1, Saxenda is believed to work in areas of the brain involved in appetite regulation, including the hypothalamus.1Saxenda for use in adults with obesity was evaluated in the SCALE (Satiety and Clinical Adiposity Liraglutide Evidence) clinical trial program. Since launch in 2015, more than 1.5 million patients have been treated with Saxenda globally.7

Saxenda is already indicated in the US for chronic weight management in adults with a BMI 30 kg/m2, or 27 kg/m2with one or more weight-related comorbidities, as an adjunct to a reduced-calorie meal plan and increased physical activity.1

For reference, alcohol has about seven calories per gram, while a carbohydrate has four calories per gram. Alcohol can also reduce inhibitions about eating, making you more likely to choose high-calorie foods if you're drinking, Chao says.

A large 2018 study of overweight and obese people with type 2 diabetes found that people who drank heavily lost less weight over a four-year period than those who did not drink and decreasing alcohol consumption may improve weight management in people with diabetes.

Mixed drinks are especially high in calories due to the sugar-laden fruit juices and other ingredients often mixed in. For example, one pina colada can contain 526 calories. Therefore, if you're trying to lose weight, it's important to limit alcohol intake to stay within your calorie goals, Chao says.

Drinking water can help you lose weight by boosting your metabolism and suppressing your appetite, thus making you feel fuller.

A small 2007 study found that drinking 500 mL (two cups) of water increased energy expenditure by 24% in overweight or obese individuals. Additionally, a small 2015 study of obese people compared participants who drank two cups of water before meals with those who did not. It found those who drank water lost an average of almost three more pounds.

If you usually drink sugary beverages, replace them with water. This will not only keep you more hydrated, but you will also no longer be consuming those calories from sugar, Chao says.

Insufficient sleep can also make it difficult to lose weight, according to a small 2010 study of overweight adults. All participants ate a reduced-calorie diet, and those who slept 8.5 hours lost an average of two pounds more than those who slept 5.5 hours.

Also, a 2008 review found that sleep deprivation can harm metabolism - the process your body uses to convert calories into energy. It found less sleep disrupts hormones that regulate appetite, possibly making people hungrier.

"If people are sleepy and fatigued, they're more likely to choose foods that are more energy dense," such as high-sugar or high-carbohydrate foods like ice cream and pizza, Chao says. And if people are awake late at night, "they might be more likely to reach for a snack or have something extra to eat," she says.

Stress makes it difficult to lose weight because it affects your metabolism. A 2011 study of obese adults found that people with lower stress levels lost more weight.

"Some of my work has shown that when people are stressed, they're more likely to choose energy-dense foods, compared to lower energy-dense foods, to help cope," Chao says. Also, if you're stressed, you may be less likely to find time to exercise or cook, instead, relying on fast food.

To reduce stress, try exercising. This will also help you burn more calories.

If you're sitting all day, you're not expending as much energy as you would if you were moving around, and "that in itself can contribute to not losing as much weight," Chao says.

There's a difference between being sedentary and being physically inactive. For example, if you sit all day for work, but also meet the recommended 150 minutes of weekly physical activity, you're still considered sedentary.

Obese people are more likely to be seated for 2.5 more hours per day than their normal-weight counterparts, according to a 2006 study. The study suggests spending 2.5 more hours - separate from exercise - standing and walking per day to help fight obesity. This could include holding work meetings or watching TV while standing.

If your weight loss has stalled for no apparent reason, you've hit a plateau. As you lose weight, your metabolism starts to decline, so you burn fewer calories than you did initially.

People often hit a weight loss plateau at around six to 12 months, Chao says. "Usually, what I recommend people start with is to self-monitor again, to track their food intake as well as their physical activity, and to track it pretty closely." That way, they can make modifications, she says.

To get past a plateau, you may need to increase your exercise. "We know that having higher amounts of exercise is really important for keeping weight off long term," Chao says.

If you need help losing weight, you can see a doctor or dietitian who specializes in weight loss. They can help you find a plan that works for you. "A lot of long-term weight loss is really about being able to adhere to something that they'll be able to stick with for their whole lifetime," Chao says.

Some underlying medical conditions can make weight loss more difficult, such as hypothyroidism - when your thyroid isn't functioning properly. Certain medications, such as some steroids and antidepressants, can also make weight loss challenging, Chao says. Your doctor can treat these conditions or change medications.

Sometimes, you're taking the right steps to lose weight, but you might not realize how sleep or stress are preventing your progress. By paying attention to your daily activities - what you're eating and drinking, how much you're moving, your feelings, and your sleep schedule - you can get back on track for weight loss.
